As part of our School Improvement Plan, we are required to document all professional development activities of our staff. I have attached a form for you to record your activities which will be collected each marking period to keep with school documents. Record ALL workshops and P.D.s you attend whether they are through the district, RESA or some other venue. You should be able to use this record with MDE to get credit for your activities toward renewing your certificate.
Save this form as a word document and then you can update as you go. Then just save the changes. When it's time to submit, you'll be ready.
